Nurse-Family Partnership® is a nurse home visiting program that empowers first-time parents to transform their lives and create better futures for themselves and their children. The program focuses on families with the greatest social and economic needs and provides support during the critical early years of life.

Key Features:

  • Supports families facing overlapping barriers to care, including limited access to health, social, and economic resources
  • Serves first-time expectant parents from early pregnancy through the child’s second birthday (the first 1,000 days)
  • Provides trusted information and resources to help parents build healthy, stable futures

How does it work?

  • Nurses partner with the mom during the first or second trimester of pregnancy, and home visits are available to the family until the child's 2nd birthday.
  • Moms learn about pregnancy, childbirth, breastfeeding/chest-feeding, child development, and nutrition.
  • Public Health nurses support families to improve their health, education, and economic self-sufficiency.
  • Partners, fathers, family members, even close friends are welcome to participate in nurse visits at the discretion of the primary client, the pregnant mom
  • Parents receive a certificate of completion on graduation from the program
  • All services are voluntary and at no cost to you.

How do I qualify?

  • First-time mom or pregnant person
  • Currently pregnant
  • Eligible for Medi-Cal
  • Resident of Santa Cruz County
